6-12th Grade Student Survey Information

Idaho Youth Survey – Parent Permission Required

Dietrich Schools will be participating in the Idaho Youth Survey for students in grades 6–12. This survey helps us better understand the needs of our students, including areas we may not otherwise be aware of. The results can also support opportunities for funding that directly benefit our students and programs.

Parents/guardians must provide permission for their child to participate. Copies of the survey questions are available upon request from Superintendent Shaw or Principal Crystal Hampton.

Please note:

  • Students are not required to answer every question.

  • Participation is voluntary, and parent permission is required.

  • At least 65% of students must have permission to participate in order for the district to administer the survey.

How to Complete the Permission Form:

  • Online: Parent Permission Form

  • Paper copy: Available upon request to be sent home at your convenience.

  • Parent-Teacher Conferences: Forms will also be available to complete electronically or in paper form.

We appreciate your support in helping us meet this participation threshold so we can better serve our students and access potential resources for their benefit.
